A girl from the working class named Poonam resides in Madhupur, a small town. Her uncle Krishnakant has filled the hole left by the death of her parents in her life ever since she was a young child. But because their own daughter Rajni has a darker skin tone and is less charming than Poonam, Rama, his wife, is envious and refuses to accept Poonam. Rajni and Poonam nevertheless become close and identify as sisters. A well-known businessman in New Delhi named Harish Chandra Bajpayee has two sons: the newlywed Sunil and the well-mannered and educated Prem.
Bhagat, a goldsmith and Krishnakant's friend, is won over by Poonam's straightforward and loving personality and accepts her wedding proposal for Prem. Harish offers his perspective on the idea. Prem is hesitant and believes he should put his profession first. Harish persuades him to meet Poonam before making a choice. Prem concurs. They pay the Mishras a visit and allow him to get to know Poonam. Instantly drawn to one another, Prem and Poonam decide to wed and become engaged.
Prem and Poonam get to learn about each other better thanks to Krishnakant's invitation to the Bajpayees to stay at their summer home in Som Sarovar. The two experience the most enchanted and passionate time of their lives during which they develop a loving attachment. Later, Harish and his family go back to their house so they may quickly attend a work meeting. Prem and Poonam correspond by phone and writing. Prem joins the company and undertakes an important project in Japan.
A celebration is arranged to honour Poonam's first visit and Prem's profitable business venture after the family surprises her with her arrival when they return. An incensed and envious Rama hides in her room throughout the festivities. She is found by Krishnakant, who confronts her aggressively and calls her out for refusing to accept Poonam despite her lifelong attempts to win Rama's love. She is the only individual who has ever had an issue with Poonam, he tells her. However, a fire starts at the Mishra house two days before the wedding. Poonam manages to escape in time, but she realises Rajni remains trapped and rescues her while suffering severe burns.
A repentant Rama falls down after learning Poonam's condition and the fact that she was hurt while saving Rajni, recognising how cruel she had been to Poonam. Prem receives a heartbroken call from Bhagatji just as he is about to head to Madhupur for the ceremony. He is ready to sign the form authorising her surgery when he starts to cry and is unable to. Prem shows in, ready to wed Poonam despite her wounds, and he brings the best medical professionals from Delhi. Before her operation, he gets married to her informally. The hospital successfully operates on Poonam with the help of the Delhi doctors. After getting married according to tradition, Poonam and Prem return home to start a new life. Prem takes it upon himself to treat Poonam's burns on their wedding night. The film ends with them sharing an intimate moment and the beginning of their married life.
Vivah was the fourth film that Shahid Kapoor and Amrita Rao did together. It was also an unexpected hit, becoming one of the highest-grossing movies of 2006. It was also the highest-grossing film of both Shahid Kapoor and Amrita Rao at the time.
